1. Agreement for Professional Services

The agreement for the delivery of professional services is strictly between you and the dental practitioner. You acknowledge that your medical aid, if applicable, is not contractually bound to the dental practitioner.

You agree to:

• Take full responsibility for following up on claims.

• Settle the account in full within 60 calendar days from the date of service, regardless of whether payment has been received from your medical aid.

2. Medical Aid Billing Consent

If you have a valid medical aid, you authorise the practice to submit the account, including ICD-10 codes, to the medical aid for payment processing.

3. Legal and Collection Costs

In the event of legal proceedings, you agree to pay:

• All attorney and client fees,

• Collection commission,

• Tracing fees,

• Any other costs associated with debt recovery.

4. Payment Terms for Instalments

If you choose to pay off your account in instalments, you undertake to make monthly payments on or before the 1st of each month, until the account is fully settled.

5. Legal Address for Communication

You confirm that the address you have provided serves as your domicilium citandi et executandi—the address at which all legal notices and communications may be delivered. You will notify the practice in writing if your address changes.

6. Declaration
